Address and phone numbers of two alternate contacts who do not currently live with you or with each other
Social Security Number or driver's license (Social Security Number is optional; used only to help us locate you)

Help Save A Life....Make A Difference!
The only hope for Easley residents Robin Burns Whittemore and Gladys Murphy to escape a premature death from Leukemia is to have a bone marrow transplant. Unfortunately, their family members are not a match and it is up to the kindness of the community to help save their lives. Even though there are millions of people registered to donate, nationwide, no perfect match has been found for either woman. WE NEED YOU! PLEASE, will you have one small vial of blood drawn and agree to be a committed member of the National Marrow Donor Program? YOU could be the only person in the world to match Robin, Gladys or some other person in the state, U.S.A. or even in another part of the WORLD. For questions you might have about details of becoming a donor you may call 1-800-Marrow-2, visit the National Marrow Donor Program's website www.marrow.org or e-mail the chairman of this drive, Julie Capaldi.

- Requirements: Must be between 18 and 60 years old...and in general good health (no cancer, heart disease or diabetes). There is a one-time tissue typing fee of $65, however, you will never have any other expenses associated with being a bone marrow donor. This $65 fee is fully tax deductible. If you would like to be a donor but can't afford the fee, please notify Julie Capaldi at (864) 420- 1635 because some donations have been made to test people that can't pay or need some assistance in paying the fee. WE NEED YOU....PLEASE ASK ABOUT THIS ASSISTANCE...DON'T LET MONEY STOP YOU FROM BECOMING A DONOR.

- 1. Print and send in form below, (to Julie Capaldi, Volunteer Coordinator, Pickens County Marrow Drive, PO Box 96, Easley, SC 29641), with a check (if you can) made out to "HENDRICK MARROW PROGRAM" along with your name, address and phone # by November 4th and be tested on Nov. 18th at Easley Presbyterian Church by The Red Cross for The National Marrow Donor Program.
- 2. Send to to Julie Capaldi, Volunteer Coordinator, Pickens County Marrow Drive, PO Box 96, Easley, SC 29641. If you can't be tested, consider sending a donation so that someone else can be tested.
- 3. If you own a business, agree to pay to have your employees tested if they would like to become donors. Your company will be publicly recognized as a part of this county wide effort to save lives.
- 4. TELL EVERYONE YOU KNOW ABOUT THIS MARROW DRIVE AND ENCOURAGE THEM TO PARTICIPATE!
